This November, students will be completing the Panorama Student Perception Survey in their homeroom classes. To review the survey questions on the HIDOE website, please click on the following link:
Please read over the attached PDF letter regarding the Panorama Student Perception Survey. Included in the attached PDF is a declination form. The declination form is required only if parents do NOT want their child to participate.
If parents would like to submit a declination form, only hard copies are accepted. The form to NOT have the child participate may be printed at home and sent in to their child's homeroom teacher. Alternatively if a printed copy of the 3rd page is needed, please come to the front office, 7:45 am - 3:30 pm, to complete and submit the form. One must be completed and submitted for each child. Last day receive a signed hard copy of the declination form is Friday, November 4, 2022.
